Crown Health is accepting applications for CMA’s and CNA’s in a diagnostic testing office in Lexington, KY! This full time opportunity is Monday-Friday, 8am-5pm with a half-hour lunch break and is available immediately! Pay with CMA certification is $17/hr and pay with CNA certification is $15.50-16/hr.
This career opportunity is temp with intent to hire. After successfully completing the required hours, you may be considered for permanent placement with the client company!
As a CNA or CMA, you can expect to:
• Responsible for registry of chronic care management (CCM) patients.
• Validates enrollment of CCM patients based on provider request
• Conducts minimum of one 20 minute of telephone or in-person counseling and education per month to each CCM patient on roster.
• Complies with documentation requirements of the Chronic Care Management program by carrying out the care plan with the patient, family/caregiver(s) and providers and recording in the EHR or patient’s physical chart.
• Monitors adherence to care plans, evaluates effectiveness, monitors patient progress in a timely manner, and facilitates changes as needed.
• Creates an ongoing process for patient and family/caregivers(s) to determine and request the level of care coordination support they desire.
• Facilitates patient access to appropriate medical and specialty providers.
• Work closely with in-office providers to manage the day to day calls involving: symptom control, medication management, and provide patient and family education
• Coordinates continuity of patient care with external healthcare organizations and facilities including from the primary care provider to a specialty care provider.
• Supports patient self-management of disease and behavior modification interventions.
• Provides patient health counseling, education and instruction.
• Provides backup support to Remote Patient Monitoring.
• Performs any overflow functions deemed necessary by the Supervisor or Team Lead.
Duties All MAs Must Be Able to Perform Efficiently:
1. Manual blood pressure
2. Rooming a patient
3. EKG
4. Placement of a variety of cardiac monitors on patient
5. Downloading medical data
6. Medication refill request
7. Hospital transfer
8. PT/INR
9. ABI testing
Great candidates will have the following experience, skills, or education:
High school diploma or GED required.
Active CNA, SRNA, or CMA required.
Minimum of 1-3 years work experience in a healthcare setting, preferably involving patients with complex chronic disease states. Additional appropriate education may be substituted for 1-3 years of healthcare experience.
Strong working knowledge of chronic disease states including hypertension, chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and congestive heart failure and basic medical management of these states.
Basic computer skills including previous work with an electronic health record (EHR).
