Job Description

Crown Services Healthcare is proud to accept applications of compassionate patient aides interested in providing Life Skills Training to residents of group homes with traumatic brain/spinal injuries. This position will also serve as the "Floater," providing support to regularly scheduled staff and participates and engages in activities with clients and assures their Health, Safety, and Welfare at all times.


As a Life Skills Trainer and Floater, you can expect to:

  • Work at different residential locations based on a schedule determined by the Residential Manager
  • Certain activities will be scheduled and floater staff will be given notice and direction
  • When no activities are scheduled, it is the responsibility of float staff to determine activities, such as cards or movies
  • Complete and turn in to Residential Manager daily forms stating activities completed and the clients who participated
  • Floater staff are required to have vehicles hat are clean, free of trash, and in good state of repair. They are also required to carry insurance on the vehicle in which client will be transported. Documentation of the insurance should be kept current and on file in the corporate office.
  • Floater staff that transports clients will be required to complete a “Modern Care Driver Agreement” to be kept in their employee file
  • Floater staff can be required to work a regular house position if emergency staffing is required
  • Floater staff may accompany clients to the Emergency Department or Doctor as required
  • Floater staff must keep accurate account of client expenditures as outlined by Modern Care
  • Floater staff must be actively working the entire scheduled shift, not dependent on client participation
  • Must notify Supervisor and Crown Services Healthcare if absent from work, as outlined by policies
  • Must notify office if scheduled activities or clients participating are changed for any reason
  • Assist to and from toilet and other ambulation
  • Assist with personal and dental hygiene
  • Keep incontinent clients clean
  • Pass medications per ABI regulation and company policies using EMR system
  • Record daily goals and other documentation requirements per ABI regulation and company policies
  • Observe client and note physical condition
