Job Description

The Community Outreach Nurse is expected to participate in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of his or her preparation. This nurse will travel and perform in-home care while administering TB medication and treatment.

• Appropriate manner, conduct, and grooming. • Shows dependability by being punctual; maintains consistent attendance 

• Physical activities involving kneeling, balancing, lifting, pulling, pushing, reaching, standing, and stooping. 


Mandatory Requirements are: • Nursing Diploma • License in good standing on Kentucky Board of Nursing with no disciplinary action on file

Job Requirements

 • Performs nursing assessment and develops health care plans for patients seen in a Clinic Setting and In-Home Setting.

• Position does require travel to patient homes to conduct screening.

• Evaluates for TB, treats active TB, performs TB Skin Test, and TB Screening

• Inspects premises for potentially hazardous conditions or infractions of sanitary codes and regulations and makes appropriate referrals.

• Provides assessment of patient complaints and recommends appropriate actions and community resources.

• Organizes and schedules field work.

• Assists in preparing for specialized clinics.

• Assist physicians and nurses during medical examinations in preventative, therapeutic and rehabilitative procedures.

• Takes and records patient’s temperature, pulse, respiratory rate, blood pressure, weight, height, and other measurements.

• Administers medications and treatments, give immunizations, perform laboratory procedures, and provides direct and individualized nursing care to an assigned group of patients based on the application of scientific principles as directed by a licensed Physician, or Nurse Practitioner.

• Obtains capillary and venipuncture blood samples from patients.

• Obtains sputum samples from patients.

• Labels, packages and routes samples appropriately for processing.

• Provides instruction to patients regarding health education and disease prevention.

• Consults with supervisor or registered nurse in initiating assessments.

• Evaluates client or family needs for referral to community resources or agencies.

• Refers situations requiring further intervention to supervisor, physician or nurse practitioner.

• Conducts follow up consultations with patients concerning abnormal test results.

• Inventories, monitors, and control medications, and medical supplies according to established procedures. • Maintains medical records, and writes reports according to program standards.

• Records information concisely, accurately, and completely.
